The undersigned hereby makes application for credit with Allied Concrete and agrees to abide by the standard terms and conditions of Allied Concrete as follows:

1. Terms: Net Days are determined based on credit review.
2. A service charge of 1.5% per month (18% per annum) will be added to all amounts billed if not paid by the end of the month.
3. No additional credit will be extended to past due accounts unless satisfactory arrangements are made with our credit department.
4. Applicant agrees to pay all costs of collection including reasonable attorney fees.
5. Applicant authorizes Allied Concrete to investigate all references provided pertaining to the credit and financial responsibility of the applicant.
6. Purchaser certifies solvency and ability to meet obligations, providing accurate financial information to obtain credit. Applicant authorizes Allied Concrete to request credit reports on both company and personal information to investigate creditworthiness.
7. Purchaser agrees to comply with Sales Laws and confirms agreement with the Terms and Conditions of Sale and Personal Guaranty.
8. Personal Guaranty, In order to extend credit to Purchaser, hereby the Purchaser guarantees payment of all existing and future debt of Purchaser to Allied Concrete, including any costs, expenses, and reasonable attorneys' fees payable as a consequence of collection efforts. This personal guaranty is irrevocable, and absolute and continuing. Guarantor may terminate this guarantee as to any new extensions of credit by written notice sent via certified or registered mail, but shall continue to be obligated for any credit extended within ten days after Allied Concrete's receipt of such notice.
